Miscellaneous Services for Indian Passport in USA

Note: Indian government’s contract with CKGS is ending on October 31, 2020. Therefore, CKGS will accept the applications till October 14, 2020. New provider VFS Global will start accepting applications from November 2, 2020 onwards. We will update the instructions when available.

You can apply for miscellaneous services for Indian passport in the USA. The exact procedure and the documents will by the exact service you are looking for.

Change of Address
If your address has changed, and if you would want your new address to appear in your Indian passport, follow the procedure described here.

You have to apply for ‘Re-issue of Passport‘ or ‘New Passport in Lieu of Lost / Damaged Passport‘.

Under ‘Choose Your Miscellaneous Services‘, check the option ‘Do you want to Change your Address?’

Along with other documents, you must submit the proof of address. If you would like to mention the USA address in the passport, that is fine.

Change of Appearance
The following constitute change of appearance:

• Infant to child;
• Child to adult;
• Non-turban to turban;
• Turban to non-turban;
• Non-beard to beard;
• Beard to non-beard;
• Non-moustache to moustache;
• Moustache to non-moustache;
• Glasses to non-glasses;
• Non-glasses to glasses.
• Difference in physique in between the photo of current passport and photo on the passport application
• Difference in appearance due to aging in between the photo of current passport and photo on the passport application

To get a new passport due to a change of appearance, under ‘Choose Your Passport Service‘, choose ‘Re-issue of Passport‘.

Notarized affidavit is mandatory for all applicants except new born child.

Change of Date/Place of Birth

Normally, when a passport has been issued on the basis of documents provided by the applicant, the question of change of date and place of birth should not arise.

the entries in the place/date of birth columns have been erroneously made by the original Passport Issuing Authority at the time of issue; OR

To change date/place of birth in the passport, under ‘Choose Your Passport Service‘, choose ‘Re-issue of Passport‘, and under ‘Choose Your Miscellaneous Service’, choose either ‘Do you want to Change your Date of birth?’ or ‘Do you want to Change your Place of birth?’ or both, as applicable.

Before submitting the physical application to CKGS, you must visit the Indian Embassy/Consulate in person between 10:00 AM and 12:30 PM to get the approval.

The applicant should carry the following documents to the Embassy/Consulate:

• Original Passport
• Application form completed and signed
• Old/Cancelled Passports, if any.
• Revised Date of Birth/ Place of Birth certificate
• One of the following:
  • A court order issued by a judicial officer not below the rank of 1st Class Magistrate in India; (Also called Declaratory Order) or
  • An amendment issued by the Competent Authority. The Competent authority should be the same authority whose certification was submitted at the time of issue of original passport; The Competent Authority in such cases can be Municipal Authorities (for date/place of birth) or Education Board Authority (for date of birth).
  • Original passport showing the correct date/place of birth;

  If approved, Embassy/Consulate will annotate ‘CKGS to accept the application’.

  If the error is on the part of the Passport Issuing Authority, a new passport will be issued without any fee.
